The Leeds and Grenville Community Family Health Team has put a new home-based care service into action called “Aging at Home”.
The program is available for patients in Brockville and Gananoque who:
- Are 65-years-old and over
- Have a chronic disease such as congestive heart failure or chronic obstructive pulmonary disease
- Have difficulty walking to and from appointments
- Experience memory loss
- Visit the hospital emergency department frequently
- Have been recently hospitalized for chronic disease
The aim of the home care service is to provide care to patients who have difficulty accessing clinics or are at risk for hospitalization.
Home visits include physical assessments, medication reviews, disease management, and illness care.
